Three total possibilities that I can think of:
1. Nothing in particular happens to Misha
2. Misha dies
3. Misha moves or something that knocks Misha out of the picture.

Sure Misha would be out of a job, but best friends don't stop being friends just because of something like that. Think about it, for a few years now, Misha has been the closest person to Shizune. Her link to the outside world. They have a pretty deep bond to say the least. I seriously doubt that Misha would be out of the picture for those reasons. If anything, something really bad would have to happen in order for her to leave.
It's a good thing Shizune is deaf, she is the only one who can stand (not) hearing "Wahaha~!" over and over.

yeah, but she won't be needed for communication between shizune and hisao. i'm not saying that misha will be forgotten completely, she just won't be an important part to their relationship since they can communicate without her.
