Suspiciously specific denial, maybe?
I have a long, long laundry list of fetishes that are probably head tiltingly kinky at best and downright odd at worst, but people being horribly burnt is not one of them, by the way.

It's definitely, absolutely not a cooking accident. Implied, at least, considering that for all the things that traumatized Hanako from the accident, along with the bullying fallout, that if the fire started because an unexplain-able power forced her to leave the stove unattended, she would give up cooking altogether.
I think it was deliberately left vague whether the fire was Hanako's fault because it doesn't truly matter at the end. However, the bullying that Hanako might have gotten might have been directed for more than her scar (possibly children blaming her). The theories proposed works. Maybe her parents were going through a divorce and an argument ended badly, I don't know. We aren't meant to know.

The thing about her not cooking if it was her fault cooking makes a lot of sense. As she was the only one of her family that survived, there's likely to have been a certain element of 'why did I 'get to' survive and they didn't, and this leads quickly onto self-loathing and, more likely, self-blame. There doesn't even have to be a reason SHE could give for why it was her fault, aside from it just was, and I think that's what happened.

1. Ikezawa family puts on fireplace
2. Gas leak
3. Ikezawa family extinguish fire, but heat is still there
4. Ikezawa family sleeps
5. Gas contacts fire
6. Pyrotechnics
Loose possibility, no?
