The face doesn't seem to match; a very effeminate face matched with a somewhat muscular body?

It's the lighting. Look at his left (our right) side, same colour. In other pictures of him all his skin matches.

If Google Translate is to be believed, the term actress (apparently a variant that's often used in a pornographic context, according to what Wiktionary says the kanji (女優) means) is being used multiple times in her Twitter profile, so I'm thinking female pronouns are the correct ones, for what it's worth.
See the next post, as that Twitter source was apparently incorrect.
