I have a question i want to know what did hisao and hanako do on there vaction that they will take after her story ends but lilly's shows????

This leads to two different paths depending on what you choose: ruining their friendship or ruining your relationship (you can ammend the friendship later though).

The game itself doesn't tell. It's not unlikely the Hokkaido trip still happens after Hanako's good end, since the reasons for the trip were Lilly wanting to relax after a tiring plane flight and Akira wanting the place cleaned up before she went there herself.
The rest is into fanfic territory. In the Hanako epilogue I wrote, the trip takes place and is mostly still about the three of them relaxing and hanging out at the summer home with two major differences; Lilly takes the futon and gives Hanako and Hisao the master bedroom (since Hanako and Hisao are already an item by the time they arrive in the summer home and this time it's Lilly wanting to give her friends some space) and because they share a bedroom, Hanako notices Hisao forgot to take his pills before bed and points this out to him, which prevents the heart flutter the next day from taking place.
In the end though, whether it takes place and what happens is up in the air.

I've seen a few fanfics that touch on the trip in Hanako's route. Most of the ones I've seen have Hanako noticing that Hisao didn't take his medicine or reminding him to do so, and that's the mindset that I'd take. It seems like she'd want to take care of him and make sure nothing happens to him.
