I'd say Shizune is just overtly competitive. Because Lilly is effectively the opposite of competitive she gets on Shizune's nerves which makes try to force some competition.

It's because Hanako is their little moeblob (is that the term?) and that presses their white knight button.

To be fair she looks more like she's got some kind of necrosis rather than burn scars.

While normally I do not have any interest in men doing men I must confess a scene between those two would be interesting.

Which only raises more questions. What kind of mental hospital would let Hisao be left alone with her? Other than a shit one.
