There are several reasons:

• Their fathers hate each other.
• Shizune's stubbornness - caused Lilly (and everyone else) to leave the student council.
• The lack of direct communication between them. (This one is mere speculation, but it's supported by most players)

Do all the routes and you'll figure it out.

Flowcharts are only for the scenes you absolutely can't get, like Mai's good ending in Kanon (or Shiori's route for that matter ).

I played through Hanako's bad route and Rin's bad+neutral route in 15 minutes, so it's not that hard to get 100%.
