For the record, Hanako learns about Hisao's heart condition fairly early in her route. In order to get to Lilly or Hanako's paths, you have to do Mind Your Step in Act 1 and choose "Sorry, I'm not in very good condition", which leads to Hisao admitting his heart problems to Lilly.
