There is nothing in the VN about her last name, and I don't think the devs ever mentioned one.
A couple of fanfictions have adopted "Daidouji" as her last name, though not all of them have.

Uh... Yamaku is a part of that town, so yes he probably would have needed to know, or at least it would have come up the moment he looked up the address of the school.
Try getting off a plane in LA and take a taxi to "Oceanside High School - should be in a small town somewhere around here"...
